Description: The index is a 14 item approach to exploring caregivers personal choices for participating in the hospital care of older adult family members. Assessment of family preference should be supplemented with an evaluation of other factors that impact caregiving ability, including the family’s legal authority (including power of attorney status), as well as the family’s understanding of the patient’s needs and, stress level. This information can be included in the initial psychosocial assessment and updated as needed.

Scoring/Interpretation: The items require yes-or-no answers which are assigned numeric values (1 for yes, 0 for no) and summed for a total score. Higher scores indicate a greater preference to participate in care. The most important information that can be obtained, however, is how a family member responds to the items on the index; this will help determine which activities, if any, the family member wants to participate in. (There is also an item at the end of the index, which is not counted in the scoring, that addresses any other concerns the family members might have).
