Description: This is a 20-item questionnaire originally developed to identify high risk informal caregivers of Alzheimer’s patients. Has been used for screening caregivers in different settings. It has 5 domains: Personal and Social, Physical and Emotional, Value, Care recipient as provocateur, and Economic. Although it was developed from a sample of caregivers of Alzheimer’s patients, it can be used for caregivers supporting any sick or elderly person.

Scoring/Interpretation: The items are rated using a 4-point Likert scale ranging from “Strongly Agree” to “Strongly Disagree”. All the items are summed to obtain a final global score. A “Low Cost” score is 20 points whereas a “High Cost” score is 100. Subscores can be used to identify specific problems in each domain.
